About Us

Michigan Health Foundation is a non-profit 501(c)(3) organization dedicated to helping patients with limited financial resources to pay for prescribed medications. If you don’t have prescription insurance or are having trouble affording your medicines, Michigan Health Foundation may be able to help.

What we do

Michigan Health Foundation offers patient assistance programs to individuals and families with no insurance or under insured to afford medications. We believe that access to medication can mean improved patients outcomes and quality of life, and can help prevent or delay more costly medical problems. We offer financial assitance, an online resource center, and co-payments (depends if funding is available), support groups and patient matching program to buy medictions.

PATIENT ASSISTANCE

Michigan Health Foundation does not charge patients a fee for its assistance. It is not affilliated with third parties who charge a fee for assistance with enrollment or medication refills. We are here to provide assistance to people with no insurance or who do not have enough insurance and need help getting medications. You must meet the application requirements to enroll in financial assistance programs.

Chronic & Life Threatening Deseases

Michigan Health Foundation offers multiple patient assistance programs for individuals with chronic and life threatening deseases—patients with financial needs to afford prescribed medications.

Health insurance cost is often a problem to many individuals and families who cannot afford to pay insurance copayments or deductibles required to cover out-of-pocket costs for medications of chronic and life-threatening conditions, such as cancer, diabetes, heart deseas and now covid-19.

SeniorCare Assistance Program provides financial assistance to eligible seniors to cover coinsurance, co payments, health care premiums, and deductables for certain medications and therapies depending on available funding. Also if a senior is eligible for health insurance but cannot afford the insurance premium, the foundation may be able to assist with insurance premiums.

SeniorCare Outreach Program works with local community centers and assisted living facilities, limited english speakers to provide monthly education, resources and blood pressure checks. We seek to improve treatment compliance and effective positive health outcomes.

SpecialCare Program provides financial assistance to individuals with disabilities or families with children with desabilities to pay for prescription medications, buy medical supplies, equipment and assist with travel expenses to medical appointments.

HealthEquity Program connects with neighborhood groups to provide resources on patient assistance programs, health care information sessions to improve the health of underserved populations.

Our Team

The management of Michigan Health Foundation consists of five business professionals with extensive experience working in various capacities in in a broad range of industries. The governing body for Michigan Health Foundation is the board of directors who are responsible for providing policy directives and setting long-term goals. The board is supported by an advisory council consisting of members from diverse backgrounds.

New COVID-19 pandemic Job Loss Provision is available for Michigan Health Foundation’s programs to help commercially insured, eligible patients who have lost their job and are experiencing a financial hardship depending on grants avilabilities.

To apply:

  • Download Michigan Health Foundation application (below). Please note that both you and your healthcare provider must complete the application.

  • In Section 4 of the application, check the box that states, “Have you recently lost your job and are experiencing financial hardship?”

  • In the same section (Section 4), check the box that states, “If yes, please attach proof of job termination or unemployment.”

  • Fax or mail the completed application, along with your proof of job termination or unemployment, to the address or fax number indicated on the application
